If you are a current patient at our office, reordering your contact lenses is as easy as dropping us an email.  All you need to do is email us with the following information.

• Number of boxes:  Right eye_______.   Left eye_______.

• Billing: [ ] Bill me.  [ ] Bill my insurance.    (If we are billing your insurance please confirm the name of your insurance company and your account number.  This will ensure that we get your lenses to you as quickly as possible.)

• Delivery:  [ ] Ship them to my home address.  [ ] I will pick them up at the office.
